Liposuction: what it does, what it does not, and the volume question

Liposuction reshapes contour, it does not treat weight, cellulite or stretch marks — and the famous five-litre limit is a convention, not a finding

Liposuction is one of the most requested cosmetic operations and one of the most consistently misunderstood. It is a contouring procedure: it removes localised fat deposits to change a shape. It is not a treatment for being overweight, and the things patients most often hope it will fix are frequently things it does not affect at all.

What it does not do

Three limits are worth stating before anything else.

  • It is not a weight-loss treatment. The fat removed in a typical procedure is a modest weight, and liposuction does not address the metabolic or dietary factors behind weight gain.
  • It does not remove cellulite. Cellulite is a feature of the fibrous connections between skin and underlying tissue, not simply of fat volume.
  • It does not remove stretch marks, which are scars in the dermis.

It also does not tighten loose skin. Where skin quality is poor or there is significant excess, removing the fat underneath can make the looseness more visible rather than less. That is the main reason a surgeon may recommend a different operation, or a combination, or none.

The operation and the recovery

The procedure typically takes one to three hours and usually involves an overnight stay. A compression garment is then worn constantly for several weeks — this is not optional detailing, it is part of how the tissue settles into its new contour.

Numbness in the treated area is typical for six to eight weeks. Gentle exercise generally resumes around three to four weeks; strenuous exercise is usually avoided for 10 to 12 weeks. Bruising and swelling settle over a period that can extend to six months, which means the contour you see at eight weeks is not the contour you will have at six months.

Guidance for flying after liposuction is generally 5 to 7 days, though your operating surgeon's clearance is what governs.

The volume question

You will see 5,000 mL described as the threshold for "large-volume" liposuction, often as though it were a safety law. It is a convention. The American Society of Plastic Surgeons states that there is no scientific data supporting an absolute cutoff, and that a safe volume is relative to the patient's body weight rather than a fixed figure.

That said, the volume removed does track with risk. In a series of over 4,500 patients with a mean of roughly two litres removed, the overall complication rate was 1.5 per cent — but 3.7 per cent above five litres compared with 1.1 per cent at or below it. The threshold is arbitrary; the gradient it sits on is real.

The most common complication is seroma — a collection of fluid requiring drainage. Serious risks, though uncommon, include pulmonary oedema and pulmonary embolism, and they are the reason fluid management and volume limits are taken seriously by careful surgeons.

Combining procedures multiplies risk

Liposuction is often proposed alongside other body-contouring surgery, and the combination is where the risk profile changes most sharply. In abdominoplasty data, the major complication rate for abdominoplasty alone was 3.1 per cent, rising to 10.4 per cent when it was combined with liposuction and body contouring. Combining more than triples the risk. For context, the major complication rate for abdominoplasty overall was 4.0 per cent versus 1.4 per cent for other cosmetic procedures.

This matters particularly in health tourism, where the pressure to do everything in one trip is structural rather than clinical. A single longer operation is convenient and cheaper in travel terms. It is not equivalent in risk terms, and a surgeon who agrees to combine several procedures without discussing that trade-off is not giving you the information you need.

If abdominoplasty is on the table, its own recovery is substantially longer than liposuction's: two to five hours of surgery, a few nights in hospital, four to six weeks off work and off exercise, a compression garment for six weeks, scars that fade over 12 to 18 months, and numbness that can last months to years.

Questions worth asking

Ask what volume is planned and how that relates to your body weight. Ask what will happen to your skin in the treated area, and what the surgeon's assessment of your skin quality is. Ask whether anything is being combined and why, and what the combined risk looks like compared with staging the procedures. Ask who manages you if a seroma develops after you have flown home, and whether that follow-up is arranged locally.
