On the face, the direction a hair leaves the skin matters more than the number of grafts — and transplanted scalp hair keeps behaving like scalp hair
Facial hair transplants are judged at conversational distance, in daylight, by people looking directly at you. That is why the technical priority shifts. On the scalp, density does most of the work; on the face, a graft placed at the wrong angle is visible even when everything else is correct. This article covers what determines a natural facial result and what the published planning figures actually are.
Why the angle changes on the face
Scalp hair leaves the skin at a relatively steep angle. Facial hair does not. Beard slits are made at roughly 10 to 20 degrees to the skin — far flatter than anything used on the scalp — because that is how native beard hair grows and because a hair emerging too steeply reads as a bristle rather than a beard.
Eyebrows are flatter still. The direction changes across the brow: near-vertical at the medial head, turning progressively horizontal through the body and tail, with the angle kept close to flat against the skin throughout. Brows are built in five to seven arched rows across three zones — the head accounting for 10 to 15 per cent of the length, the body around 60 per cent and the densest region, and the tail 20 to 25 per cent — with grafts spaced roughly 1 to 1.5 mm apart.
Getting direction wrong is one of the recognised complications of facial transplantation, and it is difficult to correct afterwards because the grafts have to be removed before new ones can be placed.
Planning figures for eyebrows
Eyebrow work uses small numbers of grafts, almost entirely single hairs. Typical planning figures are 70 to 90 grafts per brow for a female brow and 150 to 200 for a full male brow.
Growth follows a slower and more staged curve than the scalp: roughly 50 to 60 per cent of grafts growing by three to four months, a further 20 to 25 per cent by five to six months, and the final 10 to 15 per cent by seven to eight months, with the result generally complete around eight months. Expected graft loss is 5 to 10 per cent, which is why more than one session is normal rather than a sign of failure — in reported practice, 14 per cent of patients had one session, 77 per cent two and 9 per cent three.
There is one consequence patients should decide about before booking, not after: transplanted scalp hair keeps scalp growth behaviour. It does not stop at brow length. Eyebrows built from scalp hair need trimming roughly every two to three weeks, permanently. Some people find this trivial; others find it an unwelcome routine, and it is better to know in advance.
Planning figures for beards
Beard work uses mostly one- and two-hair grafts, with typical planning ranges by area:

Native beard densities are useful as anatomical references when planning distribution, though they are not one-session targets: cheek 45 to 50 FU/cm², chin 75 to 80, lateral moustache 85 to 90 and central moustache 150 to 160. The face is not uniformly dense, and a result that is evenly dense everywhere looks artificial.
The timeline is longer than most patients expect. Redness and swelling last around two to three days. The transplanted hairs shed and regrowth begins from three to four months, but the overall result is generally assessed at 10 to 14 months.
Complications specific to the face
Three problems occur on the face that are rare on the scalp:
- Cobblestoning — small raised bumps where grafts sit slightly proud of the skin, particularly on the chin and central lower lip, where the skin is thicker and less forgiving.
- Wrong graft direction, as described above.
- Post-inflammatory hyperpigmentation, more likely in darker skin, where healing leaves temporary or lasting dark marks.
Folliculitis also occurs, as it does on the scalp. None of these is common, but all of them are visible, which changes how much weight they should carry in your decision.
Where the donor hair comes from
Facial grafts are usually taken from the scalp donor area, which means facial work draws on the same finite supply as any future scalp transplant. If you have pattern hair loss and are considering both, the sequence and the budget of grafts should be planned together rather than separately. Beard hair itself is sometimes used as a supplementary donor for the scalp — the reverse direction — which is another reason to plan the whole picture at once.
